Health Hero recently went live with its Patient Engagement and Social Determinants of Health (SDoH) solution on the Epic App Orchard. With this application, they seek to enable payers and providers to manage patients’ social determinants of health and patient engagement communication. The solution allows them to create engaging campaigns that are omnichannel, disease-specific and targeted towards their social health needs.

Medicaid and Medicare are also on the current scope for Health Hero. CMS approved new CPT codes for Remote Patient Monitoring that became active in 2019. Within CPT, there are very critical ICD codes for Health Hero to cover; Z55-Z65. These codes allow them to cover very specific needs for patients, that are now covered via their current health insurance policy.

Personalization of health care is key. Health Hero wants to disrupt “personalized healthcare” by delivering social determinants of health. “Not everyone has the same needs. Some have resources and some lack of them. However, regardless of the socioeconomic status, all patients have different needs that have to be addressed. We live in a world now where everything is connected and personalized. SDoH is the future of personalized healthcare,” Diaz said about the future of SDoH.

Health Hero is now leading the market on SDoH and patient communication. Their intuitive platform integrates with all major SDoH directories and seeks to keep expanding its integrations in the near roadmap. Additionally, the company is working on additional partnerships that will allow them to inflate the delivery and implementation of the solution via Epic’s Electronic Health Records system.

Health Hero has shown to date more primary care use, reduced hospital readmissions, and increased SDoH services delivery to major hospitals and hospital systems in the US.
